Event Waiver and Release of Liability Form

All events hosted by Kākou Club require registration, which can be found on the events page. All participants and/or their parent/guardian must read and review the below consent agreement and acknowledge they have done so by checking the “yes” box found on the sign-up form on the event detail page.
